5. Caput Medusae & Varices

Portal hypertension forces blood to find alternative routes back to the heart. One visible route is through veins in the abdominal wall — creating twisted, swollen veins around the navel known as Caput Medusae.

Internally, blood may divert through the esophagus forming esophageal varices. These varices are fragile and can rupture, causing dangerous, life-threatening bleeding.

6. Spider Nevi

You may see spider-shaped clusters of blood vessels under the skin (spider nevi). One or two can be normal, but three or more suggest elevated estrogen, possibly from liver dysfunction. These lesions blanch (disappear) if you press on them.

7. Palmar Erythema

Notice reddening of the palms, especially in thenar and hypothenar areas (sides of palm). This is called palmar erythema. It may occur with high estrogen levels—when the liver can’t break down estrogen properly, levels build up and dilate blood vessels.

8. Muscle Wasting

Liver disease impairs the body’s ability to process nutrients, proteins, and energy. Over time, muscle gets broken down for fuel. You may observe muscle wasting around temples, palms, or thin limbs.

9. Dupuytren’s Contracture

In the palm, fascia can thicken and tighten, causing finger contractures (especially 4th and 5th digits). This is Dupuytren’s contracture. Though genetics play a large role, it is more common in people with chronic liver disease.

One simple test: the “tabletop test” — lay your hand flat on a table; if your palm can’t fully touch, contracture might be present.

10. Asterixis (Liver Flap Tremor)

Hold your arms out straight with wrists extended (like someone checking you for reflexes). In advanced liver disease, patients may show an involuntary flapping tremor called asterixis.

This reveals a brain-liver connection: when the liver fails, toxins like ammonia accumulate, affecting brain function in a condition called hepatic encephalopathy.

11. Fetor Hepaticus (Liver Breath)
